Community Services for Seniors and Adults with Disability in the Toronto Region

Reconnect Community Health Services offer a range of services and programs which allow individuals to age at home with dignity and continuing independence. 

Their services and programs address the needs of seniors and/or adults with disabilities at various points along the continuum of care from the most robust to frail adults and adults with Alzheimer’s disease and/or dementia.
